Have you seen the prediction form on this website? It's all you need.

In general, your prediction should be as specific as possible. If you have a range in mind, say so. Mag 4-5 is fine but don't expect credit for a 3.5. Location is best (for me) given as a lat-lon point and a circle radius OR a lat-lon point followed by range size (defining a lat-lon box). Same for date/time.

In your particular case, you can't be this specific, so if you list several possible locations they will be considered separate predictions. Thus, hitting one does not give as much credit since the misses count against you.
