Here's some idea's on what needs to be posted:

Date & Time of ear tone.
Which ear it was received in.
Length of sound in seconds.
Volume of sound & quality. ie: High pitch, very loud.
General vicinity ear tone person resides. (like LA vicinity if the person does not wish to provide city of residence.)
Impression of possible epicenter, if known.

Can you build a form for it, like the other prediction form? Or can the current prediction form include this as an addendum?
